FAQ

Frequently Asked Questions

1. Do I need a referral to get started?

No! Our patients do not require a referral from their doctor nor do they need to see a doctor before they start with us. We are able to fully assess, treat and make any recommendations for further testing or seeing a doctor based on our evaluation.

2. Do I have to be in pain to be a client at Forme?
Anyone can be a client at Forme. We offer a variety of services to cater to the needs of anyone who is looking to take care of their body and health. If you want to improve mobility, strength, balance, learn how to exercise/move, and more Forme can help. Pan is just ONE of the things we can help with.

3. I just tore my ACL but I'm waiting for surgery, should I do PT?

We strongly believe and the research supports a positive influence of Prehab for ACL tears on post-operative outcomes. There is a lot that can be and should be done to properly prepare for ACL surgery and maximize your potential for return to sport/activity after surgery.

4. What can I expect during an evaluation?

We will perform a comprehensive whole body assessment to understand your symptoms/complaints. We pride ourselves on getting to the root cause of why you are feeling what you are feeling and not just slapping a bandaid on the symptoms. This is a movement based assessment and at the end you can expect to have good understanding of what the plan is, what to expect going forward, and any next steps in order to progress towards your goals.

5. What should I wear?

Something flexible and feels comfortable to move in. Ideally, also something that allows us to expose the primary area that you may be coming to see us for (i.e. for knee pain shorts is ideal, for a shoulder injury a tank top/sports bra). If you're unsure, feel free to reach out to us ahead of time and ask!

6. Do you accept insurance?

Our clients may be able to use their insurance in a different way. We are an out-of-network practice that provides 1-on-1 care for an hour, therefore we do not accept insurance directly. We do accept cash, credit/debit card, or HSA and FSA cards and can provide a superbill for you to submit to your insurance for reimbursement, if you have out-of-network benefits. We are unable to provide superbills for Medicare patients. To find out if you have out of network benefits, you can call the member services number with your plan and ask if you have "out of network benefits," and what they are for Physical Therapy. Forme cannot guarantee reimbursement to clients by their insurance.

7. Why don't you take insurance?

-So that we can provided 1-on-1 care with all of our clients, every time.
-We can provide the level of care you deserve-attentive, personalized, progressive

-So that we can treat as many body parts as necessary in 1 session without limitations
-So we can provide and tailor the treatments that actually work for the patient, instead of what is covered by the insurance companies
-Being direct access means you can see us without a referral from your doctor
-We provide transparent pricing and therefore you will not have to worry about unclear deductibles or co-insurance costs.
