ABOUT

Lj van den Dungen
Forme — a play on the word "form" and the phrase "for me" — was born from a desire to help people reconnect with movement that feels good, purposeful, and personal.
I’m Lindsey (but most commonly go by “LJ”), a Doctor of Physical Therapy, fitness/strength coach, and certified mat Pilates instructor. Over the past 12 years, I’ve worked with hundreds of individuals — from young athletes, to new moms, to lifelong athletes, to weekend warriors, to semiprofessional athletes — who all share one thing in common: the desire to move freely and live without pain holding them back.
Through my years in both clinical and fitness settings, I began to see the same story play out — people would work hard in physical therapy, make progress, and then get lost once insurance ended or their program was over. They wanted more than just to “get back to normal”— or what insurance defines as "normal." They wanted to thrive — to hike again, play with their kids, lift confidently at the gym, return to running or sport, or wake up without the fear of pain.

That’s why I created Forme — to bridge the gap between recovery and performance, between physical therapy and real life. I wanted to create a space where movement is built around you -your goals, your body, your lifestyle.
At Forme, we work together. You bring your commitment, readiness to invest in your health and willingness to learn; I bring my experience, guidance, and belief that movement is medicine. Whether virtually, in your home, or in the studio, my goal is to help you move better, feel stronger, and build a body that supports the life you love.

Because your movement journey should be for you — and Forme was created to make that possible.
